These report forms may be printed and used to submit results to CMPT via faxing completed forms to FAX: 604-875-4100.

Each form (except the Paper Challenge form) has a designated number that is printed at the top of the form on the left side next to the CMPT logo. Forms are numbered as follows: CMPT form number 01 - 05/version e.g. A, B, C and so on/month and/year created or revised. If a form is revised the version letter, month, and year will be changed to note the revision. For example, the Gram Smear Report form was initially numbered CMPT01/A/09/2002, but revisions to this form were made in October 2009 and the new number became CMPT01/F/10/2009.   

Forms in require the Acrobat Reader (free from Adobe). Click the printer icon on the Acrobat Reader task bar to print the page. Use links from Print from Screen if you do not want to print a pdf format copy.
