Specific Enteric Pathogen Isolated Clinical Relevancy Reporting and
Notification of Public Health
  • M021-3, M74-4 Aeromonas caviae 91% of category A laboratories received acceptable grades

Please note effective November 2004: In future CMPT stool samples with an enteric pathogen, failure to indicate Notification of Public Health will result in a downgrading of score.

A Notification to Public Health would normally be submitted.

  • M101-3 Shigella sonnei
  • M084-3 Shigella flexneri and Salmonella enteritidis
  • The enteric pathogens that are ruled out should be listed.
    M051-2 No Salmonella, Shigella, Campylobacter, E. coli O157:H7, Yersinia, Aeromonas, Plesiomonas (or Vibrio) isolated – Clinical Relevancy  Reporting 96% of category A and B laboratories received a grade of 4/4.

  • M81-3 "each laboratory must assess the prevalence of specific enteric pathogens in their geographic area as well as the patient's history to determine the selective media to inoculate routinely and the specialized media to inoculate only upon special request or when clinical circumstances suggest exposure".

 

  • M011-4 (Candida albicans) Stool sample from an AIDS patient suffering from diarrhea and esophagitis. Sample contained pure Candida albicans. A report noting Candida albicans with the comment "May be clinically significant in an immunocompromised patient" and  'negative for Salmonella, Shigella, Campylobacter, E. coli O157:H7, Aeromonas, Plesiomonas, and Yersinia' was the expected result. The enteric pathogens that are ruled out should be listed.

  • M032-2 , M22-4, M081-2 Escherichia coli O157:H7

  • M093-3, M014-3, M84-3 Pleisomonas shigelloides 94% of category A laboratories and 90% of category B laboratories received acceptable grades
  • M084-3 Salmonella enteritidis and Shigella flexneri

  • VRE Screen - M063-4 Rectal or perianal swabs may not be sufficient to capture enough cells of the organism if only present in low concentrations. Culture of stool may be required to get a more representative sample.

  • M042-3, M83-5 Vibrio parahaemolyticus As reported in the November 1998 M83-5 the routine use of TCBS agar is not recommended as cases of Vibrio gastroenteritis are generally sporadic in nature and this practice is not cost-effective. Relevant history such as seafood consumption or marine exposure to prompt a search for Vibrio was not included with this challenge.
  • M023-3 Vibrio fluvialis

  • M092-2 Vibrio cholerae non 01 non 0139
  • M063-2 , M012-3, M73-3 Yersinia enterocolitica 98% of category A laboratories and 96% of category B laboratories received excellent grades.

A pure growth (in this scenario [M063-2] 4+) of a non-lactose fermenting colony on MacConkey agar from a stool sample warrants investigation.
